Why We Built DocNow
Kashif Saeed spent years in Nashville's health-IT ecosystem watching post-acute clinicians run the same circuit: excellent at their specialty, buried by a clinical record system that wasn't built for them. The problem wasn't capability. It was infrastructure borrowed from somewhere else.
The problem we set out to solve
In 2022, Kashif and a small team of health-IT engineers spent months in the field — talking to CWOCN nurses in rehabilitation centers, LCSW directors at independent home health agencies, and PM&R physicians working SNF contracts across Tennessee and Georgia. The pattern held everywhere. Wound care specialists spending 35–40 minutes charting a 20-minute wound assessment. Behavioral health providers maintaining two separate systems because their EHR had no 42 CFR Part 2 record segmentation. PM&R teams manually re-entering MIPS denominator data every quarter into a spreadsheet, because the EHR had no quality measure logic for their specialty.
The common thread wasn't poor clinical judgment. It was EHR infrastructure built for primary care or hospital medicine — then adapted, inadequately, for post-acute specialties. The documentation load wasn't the clinicians' problem. It was the software's problem.
DocNow started from a single design constraint: build for wound care, behavioral health, and PM&R first. Not as add-ons. As the primary use case. What that looks like is what we've been building since 2022, from Nashville — a city where healthcare is the core industry, not a vertical.
